NEXT STEPS

• Contact your physician/pediatrician and let them know your concerns.

• Ask that a referral be written for occupational therapy with a diagnosis attached.

• Ask the physician to fax the referral to Connect and Grow Occupational Therapy.

• Once you have reached out to your doctor, please call Connect and Grow Occupational Therapy and let us know you’d like an occupational therapy evaluation.

• Upon calling, we will complete an intake with you and follow up with your doctor’s office if we have not received your referral.

• You will be sent a link to establish your profile on Jane.app, you will then be prompted to complete intake paperwork.

• A verification of benefits will be completed prior to your visit.
